Ohio Alternative Treatment Amendment

      It is time to report your signature gathering effort!  

1. Please be sure you place your name on the “issued to” line,  fill in the name of the petition’s county (where the signers are registered voters), fill in the beginning date when you start, and fill in the end date when you finished.
2. On the very last page:  PRINT your name under the statement of circulator.  Write the number of signatures obtained. (Count all filled in lines, even those that are crossed off or those you think have a mistake this is what cost us in the first round.  Leave the “employed by” blank, as there are no paid petitioners on this project
3. Sign and give your FULL permanent address.
4. Please mail your petitions as you complete them to Ohio Alternative Treatment Act, PO Box 42042, Cincinnati, Ohio 45242

6. Also, if you are not finished collecting, please send an email to This e-mail address is being protected from spambots. You need JavaScript enabled to view it. with an estimate of how many signatures you have collected so far so that we have an idea of how many we have so far. 

We are going to continue collecting for 2013!

If we can show grassroots activity collecting maybe over 50,000 signatures, we may be able to get funding for 2013.  This will help in obtaining "paid" signature gatherers to ensure success for a 2013 ballot.
